본 연구는 직장 내 공기에 의한 자화율의 차이가 ADC 값의 계수화에 미치는 영향을 평가하고자 하였다. 연구를 위해 선암종으로 확진된 환자 50명을 대상으로 선별하였으며, 모든 대상자는 직장 내에 초음파 젤의 주입 전과 후를 나누어 확산경사자계(bvalue) 50과 1,000을 각각 적용한 DWI를 이용하여 영상화하였다. 획득한 DWI는 두 명의 관찰자가 각각 독립적으로 직장 내 종양 조직에 관심 영역을 설정하여 ADC 값을 측정하였다. paired T-test를 이용하여 초음파 젤 주입 전·후의 측정된 ADC 값의 변화에 대한 상관관계를 평가하였고, 급내상관계수(ICC)를 이용하여 두 관찰자 간 측정값의 신뢰도 및 일치를 평가하였다. 연구결과, 두 관찰자 모두 젤의 주입 후 ADC 측정값이 증가하였다. paired T-test 결과, 젤의 주입 전․후 ADC 측정값의 변화는 통계적으로 유의하였다(p < 0.05). 또한, ICC 검증 결과에서 관찰자 간 ADC 측정값의 일치도는 통계적으로 매우 유의하였다(p < 0.05). 결론적으로 직장 내 공기에 의한 자화율의 차이가 ADC 값의 정량적 평가에 영향을 미칠 수 있음을 확인할 수 있었다. 향후 ADC 값의 계수화를 통한 직장암의 상태를 평가할 시, 자화율에 의한 ADC 값의 변동에 대한 적절한 고려가 필요하리라 생각된다.

This study was to assess the effect of the variation in the magnetic susceptibility by intra-luminal air of rectum for quantification of ADC values. For this study, Fifty patients with before treatment rectal adenocarcinoma were selected. All patients were scanned MRI using by DWI technique before and after rectal lumen filling obtained using sonographic transmission gel. Also, DWI was imaged by
applying B values of 50 and 1,000 respectively. In both DWI, two blinded observers measured ADC values on regions of interest respectively intra tumor tissue. Correlation and Intra-observer agreement verification from measured ADC value by observers were compared using Paired T-test and Intra-class correlation coefficient. As a result, Both observers measurements ADC value increased after gel injection. In the result of paired T-test, both observers showed statistically significant changes in measured ADC values before and after gel injection (p < 0.05). In addition, In the result of ICC the agreement between observers was statistically significant (p < 0.05). In conclusion, it was confirmed that the difference in magnetic susceptibility by air in the rectum may influence the quantitative evaluation of ADC value. Therefore, When evaluating the state of rectal cancer using the ADC values, it is necessary to consider the change of ADC value by magnetic susceptibility.
